The Departments of Human Services are responsible for the payment of services to residents of nursing homes or services to the elderly or chronically ill who are in mid-level care or receiving services in their homes.  

    

With the passage of HB 2, effective July 1, 2008 the counties gained 100% liability for the non-federal share of the cost of Medicaid recipients of Intermediate Nursing Care and Home and Community Based Services.  The state of New Hampshire assumed liability for court-ordered services to juveniles, Provider Payments, Medicare Part D, Old Age Assistance and Aid to the Permanently and Totally Disabled.    

 The Intermediate Nursing Care line of the county budget represents the second most costly portion of the countys budget, behind the nursing home line.  INC funds nursing home care for Medicaid-eligible residents of private nursing homes, as well as county nursing homes, with costs split fifty percent federal share, 50 percent county share.

Some elderly persons prefer to remain at home, or in a setting less acute than a nursing facility.  Medicaid-eligible elderly and chronically ill adults may choose to receive services in their own homes or in mid-level care facilities. 

Counties have received prevention funds from the State of N.H., Division for Children, Youth and Families, called the 6% Incentive Funds.  The purpose of this funding is to encourage community programs to provide services that will prevent children and families from becoming involved in the juvenile justice system, or to provide judges with alternatives to costly placements or services.  The Administrative Rules are currently being re-written by DHHS to eliminate the counties from the receipt and administration of this funding.

  • Provide and coordinate educational programs for the Long Term Care industry.
  • Share information on best practices in meeting long term care regulations, laws and requirements.
